Thank you for your interests in my services. The following should answer general questions for establishing professional services with me. I am eager to work with you to help you gain the maximum knowledge and skills that you need to attain your health and wellness goals. Having been in practice for over 23 years, I have found that informing my clients of my policies allows you to operate within guidelines that negate any misuse of time and resources.

To set up an appointment you will need to:

1. Complete and sign the Policy for Services and scan to diana@healthtransformations or Fax to 985.893.0942

2. Complete the Patient Intake Questionnaire and scan to diana@healthtransformations or Fax to 985.893.0942

3. Pay a $75.00 deposit. This deposit will be used towards your initial consultation package.

The total cost for this initial package is $255 and includes the following:

Initial consultation: 75 minutes
2nd consultation: 45 minutes
3rd follow up via person, phone or internet: 30 minutes

**Appointments can be done in person, phone/skype consultations, or internet consultations as part of your consultations with me. Also you can request customized meals plans.

If you need additional service once we’ve completed our initial consultation package you may choose from the following. We can discuss your additional needs after your 3rd follow-up consultation.

For insurance coverage you will need to check with your insurance carrier to assess coverage for my services. If you have a medical condition, your insurance carrier may reimburse for my services; however, please note, I do not bill for insurance, but I will provide you with the necessary form to submit to your insurance carrier for potential reimbursement. When checking on coverage, if your insuracne company needs to contact my office for my credentials and licensure, please provide them with my clinic’s number. My staff is equipped with this information and will be happy to provide your insurance carrier with my credentials.
